Register a New Patient

Before beginning the registration process, please note that we do NOT accept the following insurance plans:

• Medicaid
• CHIPS
• Amerigroup
• Superior
• Star
• Trinity Care
• Aliera Care
• Tricare
• Tricare Prime
• Humana
• Aetna Premier

If your child has any of these insurance plans, please do not register but call the office at 469-296-8030 with any questions.

After registering you will need to upload a copy of your child’s current immunization record and health insurance information. 

Once you have a registered your child, a member of our team will call you within 48 hours, Monday – Friday, to setup any appointments you need.
